Request for access to personal information — Player's Card account 2875

Dear Privacy Officer,

I am writing to you as the person responsible for the protection of personal information at Playground, as designated in Playground's published privacy policy. This is a request for access to my personal information under section 27 of the Act respecting the protection of personal information in the private sector (Quebec, chapter P-39.1) and, to the extent it applies, section 8 of the Personal Information Protection and Electronic Documents Act (Canada).

I have held Player's Card account 2875 at Playground since shortly after the room opened in 2010. Please confirm the existence of, communicate to me, and provide a copy of all personal information Playground holds concerning me, including in particular: any note, flag, remark, incident record, restriction, ban, or exclusion entry associated with my account or my name; the date each such entry was created and last modified; the role or title of the person who created it; any document, report, or recording relied upon in creating it; and the current status of my account. Where a record contains personal information about a third party, I ask that the third-party information be removed and the remainder communicated to me, as section 40 of the Act contemplates.

I understand that a written response is due promptly and not later than thirty days after receipt of this request, and that access is free of charge. Please send the information to this email address, or advise me of the arrangements for consulting it. If any part of the request is refused, I ask that the refusal be in writing, state the section of the Act relied upon, and inform me of the recourse available.
